New York State requires that students entering grades K, 2nd, 4th, 7th and 10th as well as new enterers to the district have a physical on record for those years.  It is recommended that your child receive his/her physical from their private physician as he/she does know your child the best.  We will, however, have the school physician come to our building during the second half of the school year to do physicals for those children who were not able to have one done with a private physician.

You can print a copy of the district physical form from the Dover Elementary School home page.   Click on "health offices" on the left side of the page.   Click on "physicals" and print the form from there. 

This year the state has mandated that each child receiving a school physical have their BMI (Body Mass Index) done by the physician.   Please remind your child's physician to include this on the physical.
